More dental forms approved
Additional dental therapy forms are approved for inclusion in the dental record under the following conditions.
- Dental records may also include other types of medical treatment (e.g. Vet, HR office, compensation legislation, standard formulas, optional forms, and civil practices forms) if essential for documenting care and treatment.
- Where practical, provide in the right sequential sequence, the form in the authorized form (e.g. attach summaries of civil practice reports to verify EZ603, EZ603A, or old SF 603).
- Those not attached to permitted forms must be placed after the final allowed form indicated above within the dental record Jacket’s back cover (e.g., SF 522).
