Sick call treatment log

This log is maintained and updated in SAMS for each ship or activity. It contains the patient’s reporting:

  • Date
  • Time
  • Name
  • Rate
  • Division
  • Chief complaint
  • Diagnosis
  • Treatment
  • Disposition

This report is forwarded to the CO every single day.

Training log

SAMS also has a training module that records the training that medical departments go through. These entries include the:

  • Date
  • Title of lecture
  • Division of personnel attending
  • Duration of lecture
  • Number of Officers
  • Number of E-6 and below
  • (optional) location

Potable water log

Daily readings of the chlorine and bromine levels and weekly readings of bacteriological exams are required on potable water on ships and the field.

Records will be maintained IAW, Manual of Naval Prevention Medicine, NAVMED P-5010.

The recordings also go into SAMS Environmental Management Module
